CNS demyelinating plaques

Presentation

  • Age 15-50
  • Optic neuritis, internuclear ophthalmoplegia
  • Lhermitte’s, Uthoff’s
  • Fatigue
  • Sensory, motor symptoms

Diagnosis

  • MRI + gadolinium: lesions disseminated in time and space (periventricular, juxtacortical, infratentorial, spinal cord)
  • CSF: Oligoclonal IgG bands

Treatment

  • Acute: Glucocorticoids, plasma exchange
  • Chronic: IFN-B, mitoxantrone, glatiramer
